Kaiser Permanente’s home-based program allows members with a serious illness to receive care in the comfort of their own home.

HONOLULU — Kaiser Permanente in Hawaii has announced the launch of its new home-based hospice service for members on Oahu. Under its Care at Home portfolio, Kaiser Permanente will provide hospice services to individuals with serious illness, enhancing their quality of life and providing them and their families comforting and compassionate care.
“The Kaiser Permanente Care at Home hospice program will provide a much-needed option for our members who not only want high-quality care for their serious illness at home, but who also expect to receive this care from their trusted medical home of Kaiser Permanente,” said Johnnie Yates, MD, medical director, Hospice Services at Kaiser Permanente in Hawaii.
Recognizing the growing need for hospice services in health care, Kaiser Permanente created this program in Hawaii to complete its integrated care delivery model with full options for patients across their entire care journey. The care team will be based out of an office named Malama Ohana located at Kaiser Permanente Moanalua Medical Center, while visiting patients’ homes to provide care.
