Kaiser Permanente in Oregon offers subsidized health coverage through the Community Health Coverage Program, known as CHCP, to adults and children who don’t have access to any other public or private health care coverage. People must apply for CHCP through our community partner, Project Access NOW. For assistance, applicants may call Project Access NOW at 1-971-205-7676.
Program enrollees don’t have to pay monthly plan premiums, and don’t have to pay copays or out-of-pocket costs for most covered services received at Kaiser Permanente facilities.
Who is eligible?
You may qualify if you meet these requirements.
- You live in one of these counties: Clackamas, Marion, Multnomah, Polk, Washington, or Yamhill.
- Your household income is no more than 4 times the federal poverty level (for example, up to $62,600 for a single person or $128,600 for a family of 4 in 2025).
- You aren’t eligible for other public or private health coverage such as, but not limited to, Oregon Health Plan, Medicare, an affordable job-based health plan, or financial help through the Oregon Health Insurance Marketplace.
- In most cases, child dependents must be younger than 26.
- Individuals over 21 are eligible to receive the CHCP subsidy for a maximum of up to 3 years in a row. Eligible dependents can continue to receive the subsidy until they turn 21, no matter how long they've been in the program.
- The CHCP subsidy is available as a one-time resource, and all members must reapply annually. Members who do not reapply in consecutive years will lose future eligibility.

- Medical office visits (primary and specialty care)
- Emergency care
- Hospitalization
- Prescriptions
- Laboratory tests
- X-rays and other tests
- Vaccinations at no charge
- Vision care and pediatric vision hardware
- Hearing care and pediatric and adult hearing hardware
- Mental health care
- Dental care
