Community Health Coverage Program

Kaiser Permanente of the Northwest offers subsidized health coverage through the CHCP (Community Health Coverage Program) to adults and children who do not have access to any other public or private health care coverage. Applicants must apply for CHCP through our community partner, Project Access NOW. For assistance, applicants may call Project Access NOW at 1-971-205-7676.

Program enrollees do not have to pay monthly plan premiums, and do not have to pay copays or out-of-pocket costs for most covered services received at Kaiser Permanente facilities.  

Who is eligible?

To be eligible to enroll and remain enrolled, all the following requirements must be met: 

  • Live in one of these counties: Clackamas, Marion, Multnomah, Polk, Washington, or Yamhill
  • Live in a household with an income up to 400% of the federal poverty level
  • Not eligible for other public or private health coverage such as, but not limited to, Oregon Health Plan, Medicare, a job-based health plan, or financial help through the Oregon Health Insurance Marketplace

Please apply to Oregon Health Plan first if the applicant is younger than 26 or older than 55 years of age.   

Covered care

The CHCP provides primary and specialty care including the following:

  • Medical office visits (primary and specialty care)
  • Emergency care
  • Hospitalization
  • Prescriptions
  • Laboratory tests
  • X-rays and other tests
  • Vaccinations at no charge
  • Vision care and pediatric vision hardware
  • Hearing care and pediatric and adult hearing hardware
  • Mental health care
  • Dental care
