Community Health Needs Assessments

We believe in using the full weight of all our financial, technical, clinical and knowledge resources to help our members and communities realize health — mind, body and spirit — in all the places where they live, learn, work and play.

Our Community Health Needs Assessments guide our investments for the benefit of the communities we serve. They are critical avenues that enable us to engage with communities, helping us identify and measure community needs and assets so we can better tailor our investments in, and engagement with, communities. Ultimately, Community Health Needs Assessments enable us to respond to what are often the root causes of poor health — social and environmental factors deep-seated in inequity — and to do so in a way that values the wisdom and voices of our communities.

Our Community Health Needs Assessment process is a rigorous one that includes a diverse set of stakeholders. We utilize local quantitative data with information from our CHNA Data Platform and input from our community partners to help us get the most accurate snapshot of those community dynamics that are impacting health. Our assessments place a heavy emphasis on how the social determinants of health — factors like poverty, exposure to violence, lack of access to health-related resources such as affordable housing and transportation — are affecting the health of our communities.

Each of the 2016 Community Health Needs Assessments below provide valuable information about the communities served by each of our licensed hospitals, as well as communities and regions where we are actively serving our members and community partners.  These reports will be accompanied in the coming months by Implementation Strategies that lay out the concrete steps we are taking to respond to the health needs identified by our communities.

With the Affordable Care Act serving as our call and inspiration, we have sought to go above and beyond what is required of us, to truly listen to our communities, and bring a robust analysis to bear on the actions we will take to be a stronger, more engaged community partner.
