The South Sacramento Service area supports Sacramento County south of Highway 50, encompassing the cities of Sacramento, Elk Grove and Galt and also a portion of Amador County.

Community Health Needs Assessment

We are proud to share with you our 2016 Community Health Needs Assessment (CHNA) report. The report reflects Kaiser Permanente’s long-standing commitment to working with other organizations to address the most urgent health needs in the communities we serve.

Conducting these assessments and making them available to the public is also important in meeting new federal requirements outlined in the Patient Protection and Affordable Care Act. We invite you to view the Community Health Needs Assessments for each Kaiser Foundation Hospital and submit comments on our CHNA web page.

2016 Grants

Kaiser Permanente is pleased to announce the 2016 Community Benefit Grants Program for the Greater Sacramento Area. Through this annual grants program, Kaiser Permanente supports local organizations that are working to improve the health of the communities we serve.

Our 2014-2016 grantmaking supports projects addressing:

  • Access to Care
  • Healthy Eating and Active Living
  • Safety as a Health Issue
  • The 2016 grant deadline has passed.
